Good documentation is not about volume. It is about a small number of facts, written down at the right moment, in the right order.

The minimum viable record

For any meaningful clinical decision, capture:

• The finding that prompted the decision
• The options that were considered
• The option chosen and the reasoning, in one sentence
• The consent conversation, in the patient’s own words where possible
